Job summary

Nexushm in Illinois is seeking a Licensed Practical Nurse (LPN) to provide high-quality resident care, assess needs, and implement care plans in collaboration with the care team.

You will administer medications, monitor progress, educate patients and families, and participate in team care to ensure the best outcomes. Strong CPR certification and an active Illinois license are required; LTC experience preferred.
