Job summary

Trilogy Health Services, LLC is seeking an LPN to provide direct, quality clinical care and supervise nursing assistants on assigned units.

You will join a caring, supportive team at The Oaks at Jamestown in Hudsonville, MI, delivering resident-centered care, adhering to licensed scope, and supporting ongoing resident wellbeing.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s status and notify the charge nurse or physician when there is a change in condition.
• Identify resident problems and emergencies and initiate lifesaving measures if physician is unavailable.
• Provide holistic care for residents within the LPN scope of practice.
• Assist in monitoring medications, supplies, and equipment to ensure adequate on-hand stock.
• Perform administrative duties such as completing medical forms, reports, and charting.
• Report occupational exposures to blood, fluids, infectious materials, and hazardous chemicals per facility policy.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
