Job summary

Trilogy Health Services in Ohio is seeking a licensed Practical Nurse (LPN) to provide direct, quality clinical care on the assigned unit and supervise nursing assistants.

The LPN will observe residents daily, respond to changes, assist with medication monitoring, and perform necessary administrative tasks while adhering to the scope of practice. This role requires an active LPN license and CPR certification, and is based in Hilliard, OH.

Qualifications

• Must hold a current, valid state LPN license.
• CPR certification required.

Responsibilities

• Visit residents daily to assess health status and notify the charge nurse or physician upon changes.
• Identify problems or emergencies and initiate lifesaving measures when the physician is unavailable.
• Provide holistic nursing care within the LPN scope of practice.
• Monitor medications, supplies, and equipment; ensure adequate stock on unit.
• Complete medical forms, reports, and charting; perform related administrative duties as needed.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policy.

POSITION OVERVIEW

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
