Aliya Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to join our skilled nursing facility's compassionate care team. The LPN will provide total nursing care, supervise team members, and ensure adherence to facility standards while supporting residents' well-being.

This role emphasizes professional development, collaboration with the healthcare team, and education of residents and families about care plans.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies, maintain safety, and deliver empathetic care.
• Must be able to work with diverse resident populations, including elderly and mentally ill individuals.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift as an LPN.
• Participate in developing and implementing individualized care plans alongside the healthcare team.
• Administer medications and treatments as prescribed and evaluate effectiveness regularly.
• Maintain an environment that promotes residents’ physical, social, and psychological well-being.
• Educate residents and families on care plans and medical procedures.
• Monitor team member performance and assist with development, evaluation, and corrective action if needed.
• Delegate and ensure completion of assigned nursing tasks to team members.
• Stay current with facility policies, procedures, and ongoing nursing education.
• Recognize and act on significant changes in resident conditions.
• Document care, responses, and relevant information per facility guidelines.
• Act as a charge nurse: answer questions, resolve issues, and support resident care decisions.
• Follow all facility safety policies and procedures
