Job summary

Signature HealthCARE Hermitage, Hillcrest and Riverside locations are seeking licensed practical nurses (LPNs/LVNs) to provide direct nursing care, administer medications, and assist residents with daily living activities. You will supervise nursing assistants, monitor residents, and document care in the electronic health record, while coordinating admissions, transfers and discharges.

The role requires 1+ year of clinical nursing experience, and a state license.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Supervisory or charge nurse experience preferred.
• Knowledge of Federal and State long-term care regulations.
• Familiarity with EHR systems and MS Office.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) while ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents and communicate changes to physicians as needed.
• Develop and review individualized care plans with the interdisciplinary team.
• Document accurately per facility policy and procedures; coordinate admissions, transfers and discharges.
• Participate in quality assurance activities and regulatory surveys.

How you Will make a Difference:

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What you Need to make a Difference:

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
