Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) at Aspen Place Health Campus in Greensburg, IN. The LPN provides direct, quality clinical care and supervises nursing assistants on the unit.

Visit residents daily to observe status and notify the charge nurse/physician of changes; identify problems and, if needed, initiate lifesaving actions. The LPN delivers holistic care, manages med/supply inventories, and handles charting and admin duties.

Qualifications

• Must have and maintain a current, valid state LPN license
• Current, valid CPR certification required

Responsibilities

• Visit residents daily to monitor status and notify relevant staff of changes.
• Identify problems and emergencies and initiate lifesaving measures when physician is unavailable.
• Provide holistic care within the LPN scope.
• Monitor medications and supplies inventory to ensure adequate stock.
• Complete medical forms, reports, evaluations, and charting as needed.
• Report occupational exposures per facility policies.

POSITION OVERVIEW

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
