Job summary

Citadel Healthcare in Glenview, IL seeks a Licensed Practical Nurse (LPN) to care for residents in a long-term care setting. You will perform assessments, administer medications and therapies, monitor vital signs, and contribute to the care plan under physician orders.

Ideal candidates hold an LPN license in Illinois, have nursing60 relations with residents and families, and understand infection control and HIPAA requirements. Excellent communication and adaptability are essential.

Qualifications

• Nursing degree or diploma from an accredited college or university.
• Possess a current license to practice in the State as an LPN.
• 1 year of experience preferred.
• Long-Term care experience preferred.

Responsibilities

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.
• Performs resident assessments/observations and interventions, reviewing care plans to ensure appropriate care.
• Takes and records residents' blood pressure and vital signs, administers catheter care, IV therapy as ordered within license, and provides respiratory care.
• Documents nurse notes reflecting care provided and resident responses.
• Ensures environment is safe and addresses potential hazards.
• Monitors and directs non-licensed ancillary services to meet professional standards.
• Communicate/report information to residents, families, physicians, guardians, and facility staff as required.
• Ensures care plans are implemented and maintained; recommends changes to the IDT.
• Follows infection control standards and HIPAA policies.
• Performs other duties as assigned.

Job description

The Citadel of Glenview is seeking a Licensed Practical Nurse (LPN) to join their team! As a Licensed Practical Nurse (LPN), you will be caring for a group of residents and assuming responsibility and accountability for them. Nursing is provided through assessment, implementation, and evaluation of the plan of care.

Licensed Practical Nurse (LPN) Duties and Responsibilities:

• Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations.
• Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care.
• Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments.
• Charts nurse’s notes in an informative and descriptive manner that reflects the care provided aswell as the resident’s response to the care.
• Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted.
• Monitors and directs non-licensed ancillary services as deemed necessary to meet professional standards and maintain the highest practicable level of resident care.
• Communicate/report information to residents, resident families, physicians, guardians, governmental agencies, and facility personnel as required by facility policy and State/Federal guidelines.
• Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards.
• Ensures compliance with infection control standards. Immediately corrects/addresses identified instances of non-compliance.
• Follows and adheres to company policies and procedures regarding HIPAA (Health Insurance Portability and Accountability) compliance.
• All other duties, as assigned.

Licensed Practical (LPN) NurseQualifications and Skills:

• Excellent written and verbal communication skills in English.
• Ability to multi-task and remain calm under stress.
• Analytical and problem-solving skills.
• Understands and follows infection control standards, policies and procedures.
• Must be able to adapt to a flexible work schedule and occasional overnight travel.
• Ability to relate positively, effectively, and appropriately with residents, families, community members, volunteers and other facility staff.

Licensed Practical Nurse (LPN) Education and Experience:

• Nursing Degree or diploma from an accredited College or University.
• Possess a current license to practice in the State as a Licensed Practical Nurse.

Physical Requirements:

• Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, ability to distinguish smells and temperatures, all with or without the aid of mechanical devices is required.
• Limited potential for exposure to environmental hazards. Understanding and adherence to company safety standards and protocols required.
