Job summary
Signature HealthCARE of Georgetown seeks a dedicated Licensed Practical Nurse (LPN) to join our full-time day shift team in Georgetown, KY. You will provide direct nursing care, administer medications, assist with ADLs, and supervise nursing staff to ensure safety and high-quality resident care.
The role emphasizes accurate documentation, adherence to long-term care regulations, and collaboration with interdisciplinary teams to develop individualized care plans.
Qualifications
- Active LPN/LVN license in the state of practice.
- Minimum 1 year of clinical nursing experience; long-term care experience preferred.
- Experience in a supervisory or charge nurse role preferred.
- Knowledge of Federal and State long-term care regulations.
- Strong clinical observation, medication administration, and documentation skills.
Responsibilities
- Provide direct nursing care to residents, including medication administration and wound care.
- Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
- Supervise and direct nursing assistants and medication aides during the shift.
- Monitor and observe residents’ status; report changes to physician and responsible party.
- Develop, implement, and review individualized resident care plans with the team.
- Complete accurate clinical documentation per facility policy and procedure.

- Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
- Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
- Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
- Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
- Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
- Complete accurate and timely clinical documentation per facility policy and procedure.
- Coordinate resident admissions, transfers, and discharges according to established procedures.
- Participate in quality assurance activities, staff education, and regulatory surveys as needed.
- Perform other duties as assigned by the Director of Nursing or designee.
- Current, active LPN/LVN license in the state of practice.
- CPR/BLS certification preferred (American Heart Association or American Red Cross).
- Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
- Experience in a supervisory or charge nurse role, preferred.
- Strong clinical observation, medication administration, and documentation skills.
- Knowledge of Federal and State long-term care regulations.
- Effective verbal and written communication skills.
- Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
- Experience in a supervisory or charge nurse role.
- Familiarity with electronic health records (EHR) systems.
- Proficiency with Microsoft Office applications.
