Job summary

• Current registration as an LPN in good standing in the state where the facility is located.

Responsibilities

• Performs resident reviews on admission, monitors health status and documents findings in the electronic record.
• Updates the care plan with changes in resident status and communicates changes to staff.
• Administers and documents medication, treatment and procedures per physician orders and plan of care.
• Responds to and documents resident and family concerns and actions.
• Routinely performs rounds to ensure residents are in a safe and comfortable environment and receiving planned care.
• Assists in activities of daily living of residents.
• Supervises, assigns and assists in nursing activities of direct care staff.
