Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care to residents at The Heritage in Findlay, OH. You will supervise nursing assistants, monitor residents daily, and respond to changes in condition while documenting care and coordinating with the charge nurse and physicians.

Responsibilities

• Visit residents daily to observe and evaluate health status and notify changes to supervising staff.
• Identify problems and emergency situations and initiate lifesaving measures when physician unavailable.
• Provide holistic care within the LPN scope.
• Monitor medications, supplies and equipment to ensure adequate stock.
• Complete medical forms, reports, evaluations, charting as needed.
• Report occupational exposures per facility policies.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.

Qualifications

• Must have and maintain a current, valid state LPN license
• Current, valid CPR certification required
