Klarus Home Health is looking for a full‑time, part‑time, or PRN Licensed Practical Nurse (LPN/LVN) to join our dedicated team to provide community‑based care for our patients currently receiving home health services in the Englewood, Aurora, Lakewood, and/or Denver metro area.
The primary function of the LPN is to provide nursing care under the supervision of a registered nurse for patients of all ages in their place of residence. The LPN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.
Key Responsibilities
- Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.
- Coordinate care with the interdisciplinary group, patient/family, and referring agency.
- Assist in the development and implementation of patient care plans.
- Monitor patient health and report any changes to the supervising RN.
- Administer medications and treatments as prescribed.
- Educate patients and their families on health care needs, conditions, and options.
- Maintain accurate and timely documentation of patient care and progress.
Job Qualifications
- Education: Graduate of an accredited School of Vocational/Practical Nursing.
- Licensure: Current State license as a Licensed Vocational or Practical Nurse, or a multi‑state license issued by a state member of the NLC if practicing in a state member of the NLC.
- Must have a current driver’s license.
- Experience: Two years’ experience as an LPN in a clinical care setting. Home health experience highly preferred.
- EMR system: HCHB experience highly preferred.
- Skills: Proficiency in nursing skills as defined by generally accepted standards of practice; good interpersonal skills.
- Transportation: Reliable transportation and valid, current auto liability insurance.
Environmental And Working Conditions
- Work in patients’ homes in various conditions.
- Ability to work a flexible schedule and travel locally.
- Ability to lift up to 50 lbs and move patients.
