Job summary

Delmar Center for Rehabilitation and Nursing in New York seeks an LPN to join our full-time night-shift team (3-11pm). You will provide admissions, medication administration, and residents’ care planning under supervision, while collaborating with an interdisciplinary team to deliver high-quality care.

We offer shift differentials, weekend Baylor program, medical/dental/vision insurance, PTO, daily pay, 401k and Direct Deposit.

Qualifications

• LPN license (State) required
• CPR certification required
• Two years of nursing experience preferred

Responsibilities

• Admissions, discharge and transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Follow infection control, emergencies, and center policies
• Collaborate with interdisciplinary team to provide best outcomes
• Identify and report abnormal changes in residents’ condition
• Work under supervision per Nurse Practice Act and center policies
• Deliver nursing care for long-term or rehabilitative patients
• Collect patient data, observe and report pertinent information
• Implement patient plan of care and evaluate response
• Direct and supervise care by other nursing personnel in selected situations
• Maintain knowledge of documentation requirements
• Know equipment set-up, maintenance and use
• Maintain confidentiality and patient rights

Pay Rate: $28-32.80/hr

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents

• Medication requisitions with pharmacy and physicians

• Medication administration

• Following infection control, emergencies, and other center policies

• Collaborating with resident’s interdisciplinary team to provide the best outcomes

• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy

• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.

• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.

• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.

• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.

• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.

• Maintains knowledge of necessary documentation requirements.

• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.

• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State

• CPR Certification required

• Two Years prior nursing experience preferred
