Job summary

Oakwood Care Center, part of Legacy Healthcare LLC, seeks an LPN to join a friendly, supportive team on 2nd shift. The nurse will administer medications, conduct assessments, and lead care planning to ensure residents receive high-quality nursing care.

Qualifications

• As a minimum, an unencumbered State of Iowa Licensed Practical Nurse (LPN) license.
• Prior LPN experience is preferable but Legacy Healthcare LLC may invest in the right candidate.
• Strong clinical skills and ability to follow individualized care plans.

Responsibilities

• Provide medications and nursing care as directed by each resident's care plan.
• Conduct resident assessments and act as a team leader and resident advocate.

To be eligible for consideration applicants should have:
