Job summary

Sandy Ridge Center for Rehabilitation and Healing is seeking a Full-Time Licensed Practical Nurse (LPN) for the 11PM-7AM shift at its Milton, Florida facility. The center emphasizes short-stay rehabilitation and long-term care in a 60-bed setting.

The LPN will handle admissions, medications, infection control, care plans, and interdisciplinary collaboration, under state nursing act guidelines. Excellent benefits and supportive team culture are highlighted.

Qualifications

• LPN licensed by the State or Multistate License.
• CPR Certification required.
• Two Years prior nursing experience preferred.

Responsibilities

• Admissions, Discharge and Transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Following infection control, emergencies, and other center policies.
• Collaborating with resident’s interdisciplinary team to provide the best outcomes.
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy.
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.
