Job summary

Trilogy Health Services is seeking a Licensed Practical Nurse (LPN) to deliver direct, quality clinical care and supervise nursing assistants on the assigned unit.

The LPN will visit residents daily, monitor status, respond to emergencies, and ensure proper documentation and medication oversight. Located at Woodmont Health Campus in Boonville, IN, this role supports a compassionate, team-based environment with a focus on resident well-being.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents daily to assess physical and emotional status and notify charge nurse or physician of changes.
• Identify problems and emergency situations and initiate lifesaving measures if needed.
• Provide holistic care for residents within LPN scope.
• Monitor medications, supplies and equipment to ensure availability.
• Complete medical forms, reports, evaluations, and charting as necessary.
• Report occupational exposures per facility policy.

Position Overview

The Licensed Practical Nurse (LPN) is primarily responsible for providing direct, quality clinical care and the supervision of nursing assistants working with residents assigned to each unit.

Key Responsibilities

• Visit residents on assigned unit daily to observe and evaluate each resident’s physical and emotional status and notify the charge nurse, the physician, and/or the Director of Nursing when there is a change in a resident’s condition.
• Identify resident problems and emergency situations and initiate immediate lifesaving measures in the absence of a physician.
• Provide holistic care for residents within the scope of your LPN license.
• Assist in monitoring the inventory of medications, medical supplies and equipment to ensure that an adequate supply is on hand and readily available to meet the needs of the residents.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. as necessary
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidents.
