Job summary

Monarch Healthcare Management is seeking an LPN to provide direct, high-quality routine care to residents under nursing leadership. You will assist with the Plan of Care, implement delegated interventions, and monitor residents’ health data for changes.

You will also supervise unlicensed staff, coordinate with the RN and physician, and support residents’ physical, mental, and psychosocial well-being in a respectful, team-driven environment.

Qualifications

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing.
• Current Minnesota License as a Practical Nurse.
• Current CPR certification and emergency care.

Responsibilities

• Provide residents with direct routine care under nursing leadership.
• Participate in developing and modifying the Plan of Care with RN input.
• Collect health data and report changes to the RN promptly.
• Assign and monitor nursing tasks for unlicensed staff and escalate when needed.

LPN- Licensed Practical Nurse

Position Summary:

The LPN is responsible for providing residents with the highest quality of direct routine care, under the supervision of nursing leadership. The LPN participates in the development and modification of the tenant Plan of Care, implementing interventions that are delegated, ordered, or prescribed by the RN or physician. The LPN collects resident health data and compares to normal findings and the established current health status, reporting changes and/or responses to interventions in an ongoing manner to the RN. The LPN assigns and monitors nursing tasks or activities of unlicensed nursing staff through, assessment of their knowledge, skill, and capabilities, clarifying expectations, addressing behaviors, or escalating. All nursing activities are performed to support residents in achieving and maintaining the highest practicable physical, mental, and psychosocial well-being.

Qualifications:

• Completion of a professional nursing education program approved by the Minnesota Board of Nursing Current Minnesota License as a Practical Nurse
• Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care
