Essential Job Duties & Responsibilities

• Supervise Certified Nurse Aides under RN direction.
• Collect patient data, monitor conditions, and assess family dynamics.
• Contribute to care and discharge planning.
• Implement care plans and collaborate with the healthcare team.
• Evaluate care outcomes and revise plans as needed.
• Participate in staff orientation and education.
• Document healthcare delivery accurately per standards.
• Prioritize tasks and complete duties timely.
• Administer medications and treatments correctly.
• Provide shift reports and keep supervisors informed.
• Review medication and treatment records for accuracy.
• Utilize nursing skills and collaborate effectively.
• Manage diets and fluid intake documentation.
• Perform patient rounds and initiate nursing interventions.
• Embrace new tasks, generate ideas, and model organizational values.
• Engage in continuous improvement and staff education.

Other Duties

• Perform additional tasks as delegated by the RN.

Qualifications

• Graduate from an accredited LPN program with current state license.
• Two+ years of acute or rehab care experience preferred.
• Maintain current LPN license and CPR certification; ACLS preferred.
