Job summary

West State Street is seeking a Licensed Practical Nurse to provide patient-centered nursing care under RN supervision in a long-term care setting. You will evaluate residents, assist in assessments, administer medications, and supervise unlicensed staff to ensure quality care.

Requirements include graduation from an approved LPN program, current or pending LPN licensure, and current BLS/CPR certification. Competitive hourly pay and comprehensive benefits offered.

Qualifications

• Graduate of an approved school for practical or vocational nursing.
• Must be currently licensed by the State Board of Nursing or pending licensure.
• Must maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.

Responsibilities

• Evaluate residents to collect data, observe condition and report changes in condition.
• Contribute to nursing assessments and care planning.
• Administer medications and perform treatments per physician orders.
• Supervise and monitor patient care provided by unlicensed staff.
