Job summary

HCA Healthcare is seeking a Licensed Practical Nurse/Licensed Vocational Nurse to provide compassionate home health care in western North Carolina. You will work with the care team to support independence and patient comfort during home visits.

You will perform wound care, administer medications, document care thoroughly, and educate families to promote adherence and high-quality outcomes. A valid driver’s license is required for travel to patients' homes.

Qualifications

• Must hold a current LPN/LVN license.
• Valid driver’s license required for home visits.
• Associate degree or vocational training in nursing.
• Basic Life Support (BLS) certification within 15 days of start date.

Responsibilities

• Provide nursing care in the home, including wound care and medication administration.
• Document assessments, interventions, and outcomes clearly to support safe, continuous care.
• Monitor patient status and alert the care team of changes for timely responses.
• Educate patients and families to support medication adherence and independence at home.
• Collaborate with RNs and care coordinators to deliver personalized home care.

Job Summary and Qualifications

As a Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) at HCA Healthcare, you’ll play a vital role in delivering compassionate, personalized care to patients in the comfort of their homes. In collaboration with the Registered Nurse and care team, you’ll help manage medical needs and promote their independence - caring for patients like family, one home visit at a time.

Your responsibilities will include:

• Providing nursing care in the home, including wound care, medication administration, and support during procedures

• Documenting assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care

• Monitoring patient status and alerting the care team of any changes to ensure timely, coordinated responses

• Educating patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home

• Collaborating closely with RNs, providers, therapists, and care coordinators to deliver high-quality, personalized care in the home setting

What qualifications you will need:

• Basic Cardiac Life Support must be obtained within 15 days of employment start date

• (DL) Driver License

• (LPN/LVN) Licensed Practical or Vocational Nurse

• Associate Degree, or Vocational School Graduate
