Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides high quality clinical services within scope of practice per plan of care and infection control.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, communicating needs and changes in condition with the patient, family, physician and team.
