Qualifications

Responsibilities

• Provides direct patient care as defined by the state Nurse Practice Act.
• Implements the care plan initiated by the registered nurse.
• Provides accurate documentation consistent with the care plan.
• Assesses and educates patients and families regarding self-care techniques.
• Coordinates home health services and reports patient needs appropriately.
