Job summary

BAYADA Home Health Care is seeking a compassionate Licensed Practical Nurse (LPN) for a Per Diem role serving adult clients in Annandale, VA and surrounding areas. Medicare Home Health experience and at least one year of LPN clinical experience are required.

You will participate in a multidisciplinary team delivering skilled nursing care at clients' homes, performing meds, wound care, and assessments, with documentation in EMR and case management responsibilities.

Qualifications

• Active LPN license in Virginia.
• Minimum of one year of recent clinical LPN experience.
• Graduation from an accredited nursing program.
• Prior home health experience strongly preferred.
• Ability to travel to client homes as assigned.

Responsibilities

• Make home visits to clients in designated territories.
• Administer medications, wound care, treatments and procedures.
• Monitor client conditions and report changes to Clinical Manager and Client Services Manager.
• Follow up with, execute, and document doctors' orders.
• Perform client assessments and participate in case management.
• Document observations and care in EMR on a state-of-the-art touch pad tablet.

BAYADA Home Health Care is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team Per Diem . This office services our adult clients on a per visit basis in homes throughout Annandale, VA and surrounding areas.

Medicare Home Health experience is required for this Per Diem Role. At least one year clinical experience as a Licensed Practical Nurse is also required.

As a home health nurse, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

• Making home visits to clients living in designated territories.
• Performing assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitoring clients' conditions; reporting changes to Clinical Manager and Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing electronic medical records on a state-of-the-art touch pad tablet.

Qualifications of a Licensed Practical Nurse (LPN):

• A current license as a Licensed Practice Nurse in the state of VA.
• A minimum of one year of recent, verifiable experience as an LPN.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Prior home care experience strongly preferred, but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Additional Information:

Base Pay, depending upon qualifications:

$38.00-$48.00 per point.
