BAYADA Home Health Care in Rhode Island is seeking a licensed practical nurse (LPN) for home health care. You will provide one-on-one patient care in clients’ homes, following care plans and delivering high-quality support.

Qualifications

• LPN license required or equivalent certification.
• Home health care experience preferred.
• Ability to work 8-, 10-, or 12-hour shifts; flexibility in scheduling.

Responsibilities

• Provide one-on-one home health care to assigned clients.
• Follow individualized care plans and document care activities.
• Utilize electronic charting and coordinate with clinical team.
