Job summary

Ambercare Homecare in Silver City, NM is seeking a Licensed Practical Nurse to join our growing home health team. This full-time role offers Mon-Fri, 8am–5pm with rotating call weekends and coverage for Silver City and surrounding areas.

You will provide direct patient care per the plan of care, document accurately, and participate in coordinating home health services with the registered nurse and Clinical Supervisor. A state LPN license and CPR certification are required.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; community/home health or medical/surgical experience preferred.
• Currently licensed as a licensed practical nurse or licensed vocational nurse in the state.
• Possess and maintain valid CPR certification.
• Must have reliable transportation, current driver’s license and automobile insurance.

Responsibilities

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, reporting identified needs for other disciplines to the registered nurse and/or Clinical Supervisor.
• Uses and/or prepares equipment and supplies adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.

What You’ll Do:
