Job summary

Qualifications

Responsibilities

• Provide one-on-one care to clients according to the care plan.
• Document tasks and observations, and evaluate outcomes.
• Follow quality assurance policies and procedures.
• Receive clinical direction from the staff RN.
• Assist with additional duties as needed.
