Job summary

Luminis Health in Maryland is seeking a Licensed Practical Nurse (LPN) to contribute to direct and indirect patient care within a collaborative health care team. The role emphasizes accurate data collection, developing and adjusting plans of care, and safe administration of medications and pain control.

Qualifications include a high school diploma or equivalent, Maryland LPN licensure, BLS, and a preference for two years of experience.

Qualifications

• HS diploma or equivalent is required.
• Licensed Practical Nurse (LPN) license in Maryland is required.
• American Heart Association BLS certification required.

Responsibilities

• Provide direct care to patients and families in a culturally appropriate manner.
• Assess patients to collect data and identify learning needs per standards.
• Collaborate with the health care team to develop individualized care plans.
• Implement the plan of care and administer medications and manage pain.
• Organize and reprioritize patient care activities; serve as 1:1 sitter as needed.
• Evaluate plan effectiveness and recommend revisions.
• Communicate changes in patient status to the health care team.
• Participate in quality improvement, CPI, and risk management activities.
• Participate in unit shared governance.
• Participate in education and orientation of new staff.

Job description

Contributes to the provision of high-quality, cost-effective healthcare as a provider of direct and indirect patient care and by effectively collaborating with other members of the health care team. The LPN contributes to the nursing process in a team relationship with the Registered Nurse.

2. Consistently and thoroughly assesses patients to collect data and identify learning needs according to established standards and policies.
3. Collaborates with the health care team in developing and individualizing a plan of care for each patient.
4. Efficiently implements the patient's plan of care and demonstrates proficiency in medication administration and pain management.
5. Organizes and reprioritizes patient care activities and acts as a 1:1 sitter as needed.

Educational/Experience Requirements:

Required License/Certifications:

• Adherence to credentialing requirements of Luminis Health as stated in the nursing bylaws.
• Current licensure as a practical nurse by the Maryland Board of Nursing.
• American Heart Association Healthcare Providers BLS.
