Job summary

Matheny Medical and Educational Center in New Jersey seeks a Full Time Licensed Practical Nurse Coordinator to oversee healthcare needs of residents in Community Residences. The role coordinates medical care, administers medications, and ensures compliance with state regulations while collaborating with families and an interdisciplinary team.

You will supervise LPNs, schedule medical appointments, maintain accurate MARs, train DSP staff, and help with care planning to promote health and safety

Qualifications

  • High School Diploma or GED required.
  • Completion and verification of an LPN program.
  • One year of experience with people with developmental disabilities.
  • Effective English communication, both verbal and written.
  • LPN license by the State of New Jersey.
  • New Jersey driver's license with insurable driving record.
  • CPR & First Aid certification; BLS preferred.

Responsibilities

  • Provide clinical and managerial oversight of LPNs in group homes.
  • Coordinate and oversee healthcare services for residents.
  • Audit MARs and incident documentation; maintain records.
  • Schedule medical, dental, therapy, and specialty appointments.
  • Ensure follow-up care and implement physician recommendations.
  • Train and supervise DSPs on medications, infection control, and safety.
  • Monitor medication inventory, refills, storage, and disposal.
  • Ensure compliance with state licensing regulations and policies.
  • Collaborate with families, guardians, case managers, pharmacists and providers.
  • Accompany and transport residents to appointments and visits.
  • Review meal plans to ensure medically prescribed nutrition.
  • Provide hands-on care (ADLs, personal care, eating).
  • Orient new DSP and LPN staff to group home routines.
  • Educate DSPs and LPNs on residents' medical conditions and devices.
  • Confirm physician orders are implemented properly.
  • Develop staff schedules and ensure nursing coverage.

Matheny Medical and Educational Center has career opportunities available for a Full Time Licensed Practical Nurse Coordinator.

The LPN Coordinator is responsible for overseeing the healthcare needs of individuals living in Matheny's Community Residences. This position coordinates medical care, administers medications, ensures compliance with state regulations, and collaborates with healthcare providers, families, and interdisciplinary teams to promote the health, safety, and well-being of residents.

Responsibilities of this position include:
  • In conjunction with the Group Home Managers and RN Supervisor, provides clinical and managerial oversight of the Group Home LPNs and SRT LPNs.
  • Coordinate and oversee healthcare services for residents in the group homes.
  • Audit and maintain accurate and timely documentation, medication administration records (MARs), and incident documentation.
  • Schedule and coordinate medical, dental, therapy, and specialty appointments.
  • Ensure follow-up care and implement physician recommendations.
  • Train and supervise DSPs on medication administration, infection control, emergency procedures, and health-related protocols.
  • Monitor medication inventory, order refills, and ensure proper storage and disposal of medications. Follow up when necessary.
  • Ensure compliance with state licensing regulations and agency policies.
  • Collaborate with families, guardians, case managers, pharmacists, and healthcare providers to coordinate comprehensive care.
  • Accompanies and transports residents to appointments and outpatient visits.
  • Reviews meal plans to verify delivery of medically prescribed and proper nutrition.
  • Provides 'hands on care' to the residents, including activities of daily living (ADLs), personal care, and assisting residents with eating.
  • Assist the group home leadership team with orienting new DSP and LPN staff for the group home routines.
  • Conducts education with DSPs and LPNs on each resident's medical conditions, treatments, and medically prescribed devices.
  • Confirms physician orders are implemented properly and accurately.
  • Develop staff schedules and ensure adequate nursing coverage.

Position Requirements
  • High School Diploma or G.E.D. required.
  • Completion and verification of a LPN program.
  • One year of experience working with people with developmental disabilities.
  • Ability to effectively communicate in English both verbally and in writing.
  • Licensed Practical Nurse by the State of New Jersey Board of Nursing.
  • Valid New Jersey Driver’s License with an insurable driving record.
  • Adult CPR & First Aid certification required. Basic Life Support (BLS) certification preferred.
