Freedom Behavioral Lake Charles is expanding its Case Management team to support patients through treatment, discharge planning, and post-discharge coordination. The Case Manager will assess patient needs, coordinate services with physicians and nurses, and serve as a key advocate to ensure safe, effective continuity of care in our growing facility.

Qualifications

• Education: psychology preferred or nursing education.
• Experience: 1+ years coordinating utilization review and discharge planning.

Responsibilities

• Conduct assessments and assist with individualized treatment and discharge planning.
• Collaborate with physicians, nurses, therapists, families and community partners to address mental health and social needs.
• Coordinate follow-up services and connect patients with community resources.
• Advocate for patients and serve as a key communication link among patients, families, and the treatment team.

Position Summary

The Case Manager at Freedom Behavioral Lake Charles plays a vital role in supporting patients throughout their treatment journey by coordinating care and ensuring each patient receives the services, resources, and support needed for successful recovery and continuity of care.

Responsibilities

In this role, the Case Manager conducts assessments, assists with individualized treatment and discharge planning, and collaborates closely with physicians, nurses, therapists, families, and community partners to address each patient’s mental health and social needs. Responsibilities also include coordinating follow‑up services, connecting patients with community resources, and supporting safe and effective discharge planning. The Case Manager serves as an advocate and key point of communication between patients, families, and the treatment team while helping promote positive patient outcomes and stability.

Qualifications

• Education: A psychology major is preferred or nursing education
• License: Nursing License, Social Worker License, and or LPN
• Experience: 1+ years coordinating utilization review and discharge planning activities
