Job summary

Monarch Healthcare is hiring a Licensed Practical Nurse to provide direct and indirect services under supervision for crisis assessment, injections, and outpatient care. The role involves coordinating with nurses, psychiatrists, and other staff to ensure seamless patient care within our multidisciplinary team.

The LPN will schedule and triage crises, perform clinical intakes, document medical records, and educate patients about medications.

Qualifications

• Associates degree in nursing or equivalent (Required).
• High School Diploma required.
• Licensed Practical Nurse (LPN) licensure (State Board of Nursing) required.

Responsibilities

• Schedule individuals for crisis and assessment services and triage emergent cases.
• Conduct clinical intakes and coordinate with screening and referrals to ensure continuity of care.
• Document medical records to meet state and accreditation standards with providers and staff.
• Administer injections at the Injection Clinic and educate patients about medications.
• Maintain EMR documentation, check Medicaid/Medicare status, and handle mail to clients and providers.
• Attend meetings and trainings; maintain required certifications and comply with agency policies.

Job Description Summary:

The primary responsibilities of the Licensed Practical Nurse are to provide direct and indirect services, under the supervision of a Registered Nurse, to individuals in need of crisis assessment, injection clinic, or out-patient services.

What You’ll Do:

• Schedule individuals for crisis and assessment services and triage individuals who present with emergent medical and/or psychiatric problems.
• Perform clinical intakes with individuals requesting services, coordinate with screening/referral and individuals to ensure continuity of care.
• Complete documentation required to ensure that the medical record for crisis and assessment services meets state and accreditation standards, coordinating with STR, psychiatrists/nurse practitioners, providers, and crisis services staff.
• Schedule individuals for the Injection Clinic as well as doing injections of psychotropic medication, educate and support individuals about medication.
• Complete documentation required to ensure that the medical record for crisis and assessment services meets state and accreditation standards, engage with drug company representatives who supply samples or coupons.
• Deal effectively with a variety of people, which includes individuals requesting services, staff, providers, and the public.
• Input electronic medical record documentation, including checking for Medicaid, IPRS, and Medicare status of individual receiving services.
• Mail correspondence to individuals receiving services and the provider community.
• Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
• Demonstrate knowledge of and comply with all agency policies and procedures, as well as service definitions related to specific program areas
• Demonstrate knowledge of emergency procedures and assists in crisis situations.
• Complete all other relevant responsibilities as assigned by the supervisor.
• Driving and travel may be required.
