Avamere Riverpark in Eugene, OR is seeking a Licensed Practical Nurse to join our team. You will provide basic nursing care, perform vital signs, administer medications, and assist residents with daily activities under the supervision of an RN.

This full-time role offers Day and Evening shifts (6a-2p and 2p-10p) at Avamere Riverpark, with opportunities for professional development and a supportive team committed to resident comfort and safety.

Qualifications

• Must possess a nursing degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an LPN in this state.
• Must maintain and have an active CPR/BLS during employment.
• Prefer experience in long term care, geriatric rehab, sub-acute and or critical care clinical setting.
• Experience in electronic medical records documentation.

Responsibilities

• Provide basic nursing care including vital signs, medication administration, wound care, and assistance with daily activities under RN supervision.
• Record patient history and monitor changes in condition, reporting concerns to the RN.
• Maintain patient safety and infection control, documenting all care provided and observations.
• Collaborate with CNAs and other healthcare professionals to ensure comprehensive resident care.

Licensed Practical Nurse

Status: Full-Time

Schedule: Days (6a-2p) and EVEs (2p-10p)

Location: Avamere Riverpark of Eugene - 425 Alexander Loop, Eugene, OR 97401

Job Summary

Licensed Practical Nurse provides basic nursing care, including vital signs, medication administration, wound care, and assisting residents with daily activities, under the supervision of a Registered Nurse (RN)

support and comfort to residents and their families.

Essential Duties and Job Responsibilities

• Basic Nursing Care:
• Vital Signs: Accurately measure and record vital signs like temperature, pulse, blood pressure, and respirations.
• Medication Administration: Administer medications as prescribed by a physician, following facility protocols and under the supervision of an RN.
• Wound Care: Perform wound care, including changing dressings and monitoring for signs of infection.
• Hygiene and Mobility: Assist residents with personal hygiene, dressing, bathing, and mobility.
• Nutrition and Hydration: Ensure residents receive proper nutrition and hydration, assisting with feeding if necessary.

Patient Monitoring

• Patient History: Record a patient's history and current condition.
• Observation: Observe and document changes in a resident's condition, reporting any concerns to the RN.
• Communication: Communicate effectively with residents, families, and other healthcare professionals.

Collaboration and Supervision

• RN Supervision: Work under the supervision of an RN, following their guidance and instructions.
• CNA Supervision: Supervise certified nursing assistants (CNAs) in providing basic nursing care.
• Teamwork: Collaborate with other healthcare professionals to ensure residents receive comprehensive care.

Other Duties

• Patient Safety: Maintain a safe environment for residents, following infection control protocols and other safety guidelines.
• Documentation: Accurately document all care provided, including medications administered, vital signs, and any observations.
• Emotional Support: Provide emotional support and comfort to residents and their families.

Requirements
