Evergreen Health and Rehab in Winchester, VA is seeking dedicated LPNs to join our clinical team. We offer Full Time/Part-Time Day, Evening, and Night shifts with a competitive pay range and shift differentials. This role includes immediate benefits, daily pay access, and a generous sign-on bonus up to $5,000.

Qualifications

• Current LPN license in Virginia.

Responsibilities

• Provide nursing care to meet resident needs per standards and orders.
• Administer medications and treatments with precision and compassion.

We offer

Full Time/Part-Time Day (7a-3p/7a-7p), Evening (3p-11p), and Night (11p-7a/7p-7a) shifts. Available schedules may vary based on current facility needs. Rate of pay: $31.00 to $34.59 (plus Shift Diffs as applicable)

What You'll Do:

Provide nursing care to ensure resident needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines

• Administer medications and treatments with precision and compassion
• Collaborate with RNs, CNAs, and interdisciplinary teams
• Support residents’ physical, emotional, and psychological well-being
• Assist with resident comfort.
• Collect samples and monitor catheters. Provide wound care.
• Keep resident patient information confidential, follow all policies and procedures, and adhere to federal, state, local requirements, and professional standards.
• Collect, track, maintain, and review patient medical history, charts, and physical condition, including heart rate, blood pressure, temperature, and respiration.
• Help coach staff as necessary.

Requirements:

• Current LPN license in Virginia
• CPR/BLS certification (or willing to obtain)
• Previous LTC or SNF experience preferred, but new grads welcome!
• Positive attitude, reliability, and strong clinical judgment
