Job summary

Brooksby Village by Erickson Senior Living is seeking a Licensed Practical Nurse (LPN/LVN) to provide clinical care to residents in our neighborhood settings. You will work within nursing standards and Erickson Senior Living guidelines to support residents’ physical and emotional well-being.

The role requires at least 1 year of relevant experience, an active LPN license, CPR certification, and comfort with electronic medical records.

Qualifications

• Minimum of 1 year of experience in assisted living, home health, or long-term care preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• Experience with electronic medical records is preferred.

Responsibilities

• Develop relationships with residents and families, placing the resident first.
• Assess, observe, and document residents' clinical condition in the EMR.
• Update or modify care plans as needs change.
• Supervise and lead care associates.
• Administer medications timely and accurately according to orders.
• Participate in meetings related to resident outcomes and care.
• Respond to residents and families in a timely, professional manner.
• Contribute to reviews of clinical indicators (falls, wounds, nutrition, antipsychotic use).
• Support RN clinical leader with other duties as assigned.

Location: Brooksby Village by Erickson Senior Living

Join our team as a Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN). The LPN/LVN is responsible for providing clinical care to residents in the neighborhood according to the nursing standards of practice, board of nursing, and Erickson Senior Living standards.

How you will make an impact

• Developing relationships with the residents and their families using a consistent caregiver approach supporting the resident’s physical, spiritual, emotional, and clinical needs and focusing on placing the resident first.
• Assessing, observing, and evaluating resident’s clinical condition, document findings in the resident electronic medical record, and notify the provider and/or RN when necessary.
• Updating or modifying care plans as needed to reflect any changes to a resident’s care and service needs.
• Supervising and leading care associates
• Administering medications timely, accurately, and according to orders.
• Participating in meetings related to resident outcomes and care
• Responding to residents, family, or responsible parties in a timely and professional manner
• Participating in the review of clinical indicator reviews – falls, wounds, nutrition, antipsychotic use, etc.
• Other duties as assigned by RN clinical leader

What you will need

• Minimum of 1 year of experience in assisted living, home health, or long-term care is preferred.
• Licensed by State as a Licensed Practical Nurse.
• CPR certification required.
• A passion to care for seniors and be a strong resident advocate.
• Experience with electronic medical records is preferred.
