Job summary

Constellation Health Services is seeking a compassionate Licensed Practical Nurse (LPN) to provide end-of-life and hospice care in patients' homes and communities. Under an RN, you will plan patient and family care, follow care plans, administer treatments, and collaborate with a dedicated team to ensure positive outcomes.

Qualifications

• Graduate of an accredited practical nurse program.
• LPN licensed in the state of employment.
• Preferably experience in hospice, palliative or community health home care or medical/surgical settings.
• Background in clinical auditing and chart reviews is a plus.
• Wound care experience is preferred.
• Understanding of hospice philosophy and the needs of the terminally ill.
• Basic knowledge of iOS operating system.

Responsibilities

• Deliver nursing care in accordance with the State Nurse Practice Act, with a focus on end-of-life care.
• Implement current nursing practices based on comprehensive assessments and care plans.
• Maintain accurate and timely documentation of patient services.
• Provide patient and family/caregiver education and support.
• Recognize and address changes in a patient's condition.
• Collaborate with interdisciplinary teams, such as RNs, hospice aides, social workers, and more.
• Participate in Hospice IDT meetings to ensure coordinated care.
• Use and maintain equipment and supplies effectively.
• Manage pain and symptoms, evaluating patient responses to treatments.
