You will provide essential primary care services, immunizations, and health education while coordinating with RNs and physicians to improve community health outcomes. Formación

• Certificate from an accredited practical nurse program.
• Current NY State LPN license and registration.
• 1–2 years in an ambulatory setting preferred.
• Venipuncture, medication administration and immunizations.
• Basic computer proficiency, including typing.

Responsabilidades

• Administer medications, treatments, and wound care under RN supervision.
• Monitor and document patient conditions, vital signs, and progress.
• Assist with care plan implementation and collaborate with RNs, therapists, and physicians.
• Educate patients and families on post-treatment care, medications, and recovery goals.
• Maintain accurate clinical records and medication logs.
• Ensure infection control and safe protocols are followed.
• Provide emotional support and advocate for patient needs during rehabilitation.

Qualifications
