What You'll Do

As an LPN, you will provide compassionate nursing care while helping ensure the health, safety, and well-being of our residents.

Your responsibilities will include:

• Provide nursing care to assigned residents according to individualized plans of care
• Monitor and assess residents’ conditions and promptly communicate changes to the appropriate members of the care team
• Administer and assist with medications in accordance with applicable regulations, physician orders, and established procedures
• Follow the Five Rights of Medication Administration to promote safe medication practices
• Obtain, review, and transcribe physician orders as appropriate
• Reorder, receive, and maintain medications according to established procedures
• Provide personal care and assistance with activities of daily living when needed
• Respond to resident emergencies and provide appropriate follow-up
• Maintain accurate and timely clinical and case management documentation
• Complete thorough shift-to-shift communication to promote continuity of care
• Communicate effectively with residents, families, physicians, pharmacies, and other healthcare professionals
• Work collaboratively with Resident Care Associates and other members of the care team
• Provide guidance and oversight to care staff as assigned
• Assist with supervision of care staff in the absence of the Wellness Director or Assistant Wellness Director
• Help ensure residents receive care that promotes dignity, independence, comfort, and quality of life
• Protect resident privacy and confidentiality
• Follow infection prevention, safety, and emergency procedures
• Recognize and report concerns related to resident safety, abuse, neglect, or changes in condition
• Participate in required training, competency programs, emergency drills, and safety activities

Qualifications

• Previous experience in senior living, assisted living, long-term care, memory care, home health, or a similar setting is preferred
• Must be able to successfully complete required training and competency programs
• Must meet applicable health and employment requirements
• Must be able to work assigned shifts and maintain reliable attendance

Physical Requirements

This is an active nursing position and requires the ability to:

• Move intermittently throughout the workday
• Stand, walk, bend, reach, push, and pull as needed
• Assist residents with mobility, transfers, and personal care
• Lift, push, pull, move, and/or carry at least 25 pounds as required by the position
• Use mechanical equipment or assistive devices when appropriate
• Work safely with residents who may have physical, cognitive, or behavioral challenges
• Assist with emergency evacuation procedures when necessary
