Responsibilities
  • Provides direct primary nursing care to residents and supervises the care provided by certified nursing assistants.
  • Keeps administrative nursing staff informed on personnel development.
  • Provides feedback to Nurse Managers regarding performance of nursing assistance.
  • Participates in committees as assigned.
  • Demontrates ability to address service concerns and follow up timely for resolution.
  • Supports all efforts through the facility’s Service Excellence Initiative.
  • Effectively supervises and directs licensed and non‑licensed nursing personnel.
  • Responsible for developing CNA resident care assignments and other duties as needed prior to the beginning of each shift.
  • Promotes and maintains positive and collaborative relationships with physicians, physician‑extenders (NPs or PAs), lab, diagnostic personnel, and pharmacy to ensure proper and safe care of residents.
  • Promotes and maintains positive and collaborative relationships with nursing personnel, other departments, residents, families, and visitors.
  • Observes residents daily and reports any significant change of resident’s condition to the physician.
  • Completes routine nursing documentation for assigned residents.
  • Monitors to ensure that nursing care documentation is accurate, concise, and timely.
  • Administers and documents medications using the five rights of medication administration while adhering to infection prevention and control practices.
  • Provides and/or receives shift report specific to changes or concerns regarding residents to on‑coming and off‑going shift.
  • Implements pharmacy processes regarding ordering/reordering of medications, utilizing STAT/Emergency medication inventory when indicated, receiving, and returning medications.
  • Performs physical assessment for residents, recognizes normal and abnormal physiological findings based on the resident’s baseline, and reports findings as indicated.
  • Performs wound care in accordance with physician/physician‑extender orders and documents treatments completed.
  • Assists in the development of the residents’ plan of care, ensuring care is provided according to the plan.
  • Assists with updating the resident’s plan of care as indicated.
  • Keeps administrative nursing staff informed regarding evaluations of resident care.
  • May participate in rounds with physicians, physician‑extenders, other healthcare personnel to provide pertinent information regarding the resident’s care/condition and follows through with the physicians/physician‑extender orders.
  • Reviews physician’s/physician‑extender orders, lab/diagnostic reports, and contacts pharmacy regarding any change in medication.
  • Effectively communicates with family members regarding resident's care and/or condition.
  • Completes accident/incident reports and follows up; reports all accidents/incidents to administrative nursing staff and notifies physician and family.
  • Performs routine nursing skills competently and professionally, including but not limited to:
    • Phlebotomy
    • Initiation of peripheral intravenous (IV) therapy/Care of central lines
    • Wound care
    • Medication administration
    • Insertion of urinary catheters
    • Administration and management of tube feeding formula
    • Oxygen administration
    • Tracheostomy care
    • Airway management
    • Physical assessment
    • Skin assessments
  • Communicates staffing problems and needs to unit manager/supervisor.
  • Reports service and unit concerns to the unit manager/supervisor.
  • Receives and provides end‑of‑shift reports.
  • Assists in admission, transfer, and discharge of residents.
  • Reports maintenance and housekeeping needs accordingly.
  • Maintains narcotic records accurately and validates narcotic/scheduled medications before shift starts and before leaving for the day/shift; notifies unit manager of discrepancies in medication inventories.
  • Performs other duties, as assigned.
Qualifications
  • Current LPN license in good standing with the Virginia Department of Health Professions.
  • Two years of experience as an LPN, preferred.
  • Demonstrates excellent clinical, customer service, and leadership skills.
  • Current CPR Healthcare Provider certification.
Physical Requirements
  • Prolonged periods standing, walking, working, and frequently pulling, pushing, and bending.
  • Must be able to lift 25 pounds at a time.
  • Must be able to move 150 pounds with assistance.
