Job summary

Lifespace Communities, headquartered in Dallas, Texas, is seeking a Licensed Practical Nurse to join a compassionate team serving residents in a 5-STAR community. You will participate in interdisciplinary planning, document care needs, and manage medications to ensure safety and quality outcomes.

You will coordinate care, support families during changes in medication, and help maintain individualized care plans while advancing your professional growth in a mission-driven organization.

Qualifications

• Licensed Practical Nurse graduate from an accredited nursing program.
• Certifications and LPN license required by state regulations.

Responsibilities

• Participate with the interdisciplinary team to review and evaluate resident care.
• Document resident condition and nursing needs accurately and timely.
• Report pertinent observations regarding residents promptly.
• Develop and monitor nursing assistant assignments based on census and care level.
• Execute treatments and document reactions to medications and therapies.
• Generate, prepare, administer, and chart medications.
• Initiate physician orders and facilitate family communication about meds.
• Ensure care plans reflect resident individuality and are updated.
• Coordinate admissions, discharges, and transfers to deliver quality service.

• Participate with members of the interdisciplinary team to review, plan, coordinate and evaluate resident’s care.

• Establish protocol to ensure care plans are continually reviewed for updated and accurate data that represents the individuality of the resident.
• Coordinate admissions, discharges, and transfers in order to deliver quality customer service.

Qualifications

• Licensed Practical Nurse graduate from an accredited nursing program.
• Certifications and Licensed Practical Nurse license and other licensure required by state regulations.
