Cherokee Nation Home Health Services in Tahlequah seeks a qualified LPN to administer skilled nursing care in patients’ homes under RN supervision. This nonprofit agency has 40+ years of service and covers 14+ surrounding counties.

Qualifications

• Graduate of a state approved LPN program and licensed in the applicable state(s).

• Home health setting with RN supervision and physician orders.

Responsibilities

• Administer prescribed medications and treatments per physician orders.
• Monitor patient vitals and health status; report changes to RN/physician.
• Document and chart patient information accurately.
• Educate patients and families on conditions, medications, diet, and self-care.
• Collect laboratory samples as ordered.

Cherokee Nation Home Health Services in a non-profit health care agency that has been in business for 40+ years. Although, our main office is located in Tahlequah (Cherokee County), we have clinical staff that that provides health care services to 14+ surrounding counties. You do not have to be Native American to apply for CNHHS.A qualified Licensed Practical Nurse (LPN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse.Some duties for an LPN include:Administering Medications and Treatments: Give prescribed oral, topical, and injectable medications, and perform routine treatments like wound care, dressing changes, and catheter care.Monitor Patient Vitals and Health Status: Accurately measure and record vital signs (blood pressure, pulse, temperature, respiration), observe patients for changes in condition, and report and adverse reactions or critical changes to the Registered Nurse (RN) or physician.Document and Chart Patient Information: Maintain detailed and accurate patient records, including symptoms, care provided, and progress, ensuring clear communication within the healthcare team.Educate Patients and Families: Provide basic health instructions and information to patients and their families regarding their condition, medication, diet, and self-care techniques.Collect Laboratory Samples: Collect blood, urine, or other specimens for laboratory testing as ordered by the physician.QUALIFICATIONS:Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing.Minimum of one (1) year experience in nursing, preferredCurrent state Drivers LicenseReliable transportation with current auto liability insuranceBENEFITS:Monday-FridayCompetitive Salary401k with up to 3% matchAccidental Death and Dismemberment (100% paid by CNHHS)Medical Insurance (75% paid by CNHHS)Dental InsuranceVision InsuranceAflacMileage Reimbursement or company vehicle as availableMonthly Cellphone Stipend11 Paid Holidays3+ weeks of Paid Time Off
