Role Description

This is a full-time, on-site Licensed Practical Nurse (LPN) role based in Scottsdale, AZ. The LPN will provide direct patient care, including monitoring vital signs, administering prescribed medications, performing basic treatments, and assisting with activities of daily living. Daily responsibilities include documenting care accurately, reporting changes in client condition to supervising clinicians, and following established care plans and safety protocols. The LPN will collaborate with registered nurses, physicians, caregivers, and family members to coordinate care and support client comfort and well-being. The role also involves educating clients and families on basic health practices, maintaining infection control standards, and contributing to a respectful, supportive environment.

Qualifications

• Active and unrestricted Licensed Practical Nurse (LPN) license in the state of Arizona.
• Demonstrated skills in basic clinical care, including medication administration, vital sign monitoring, and routine treatments.
• Strong documentation and charting skills, with attention to accuracy and regulatory compliance.
• Effective communication and interpersonal skills for working with clients, families, and interdisciplinary care teams.
• Ability to prioritize tasks, follow care plans, and respond appropriately to changes in client condition.
• Commitment to maintaining patient safety, infection control, and confidentiality standards.
• Experience in home care, long-term care, or similar settings is preferred but not required.
• CPR certification and familiarity with basic emergency response procedures.
• Capacity to perform the physical requirements of the role, such as assisting with transfers and mobility, in a safe manner.
