Job summary

Pride Health in Rochester, MN is seeking a Licensed Practical Nurse (LPN) to support ambulatory care for a 13-week contract with potential extension. Day/night shifts from 6 AM–6 PM or 6 PM–6 AM.

The role requires graduation from an accredited PN program, BLS, and experience in Assisted Living, Short-Term Rehabilitation, and Long-Term Care; at least 1 year LPN experience; strong communication and teamwork; EMR proficiency.

Qualifications

• Graduate of an accredited Practical Nursing program.
• BLS Certification required.
• Assisted Living experience required.
• Short-Term Rehabilitation experience required.
• Long-Term Care experience preferred.
• 1 year of LPN experience required.
• Strong communication and teamwork skills.

Responsibilities

• Provide direct nursing care under the supervision of an RN.
• Observe, monitor, and report changes in patient condition.
• Assist with patient assessments and care planning.
• Administer treatments and medications as directed.
• Document patient care accurately in the EMR.
