Job summary

Live Well Homecare is seeking a Licensed Practical Nurse (LPN) to provide resident-centered care in a supportive, family-like environment. You will administer medications, monitor conditions, and document in EMRs while ensuring comfort and safety for residents.

In collaboration with an interdisciplinary team, you will help develop and implement individualized care plans, support rehabilitation and long-term goals, and educate residents and families to promote well-being and recovery.

Qualifications

• Licensed Practical Nurse (LPN) with valid licensure.
• Compassionate bedside nursing and medication administration.
• Strong communication and teamwork abilities.
