Role Description

This is an on-site Licensed Practical Nurse (LPN) role based in Philadelphia, PA. The LPN will provide direct patient care, including vital sign monitoring, medication administration, wound care, and assistance with activities of daily living in accordance with established care plans and clinical protocols. The role involves accurately documenting patient status, reporting changes in condition, and collaborating with registered nurses, physicians, and other healthcare professionals to ensure safe, coordinated care. The LPN will educate patients and families on treatment plans and health maintenance, support infection control and safety practices, and participate in quality improvement activities. The position requires consistent, reliable attendance and adherence to organizational policies, professional standards, and ethical guidelines.

Qualifications

• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania and current Basic Life Support (BLS) certification
• Demonstrated skills in direct patient care, including vital sign assessment, medication administration, wound care, and documentation
• Strong communication and interpersonal skills for working with patients, families, and interdisciplinary care teams
• Ability to follow care plans, clinical protocols, and safety procedures while exercising sound clinical judgment
• Experience in long-term care, home health, community health, or similar settings is preferred
• Proficiency with electronic health records (EHRs) and basic computer applications
• Reliability, professionalism, and the ability to manage multiple tasks in a fast-paced environment
• High school diploma or equivalent; completion of an accredited LPN program required
