Job summary

Home Sweet Home is seeking a Licensed Practical Nurse to deliver skilled nursing care in patients' homes under RN supervision. You will base care on individualized plans and document visits in the EHR.

The per diem schedule offers flexible hours with a supportive team focused on compassionate, patient-centered care. Travel to Sun City and surrounding Maricopa areas is required. Arizona license—home health experience preferred.

Qualifications

• Must hold Arizona LPN license in good standing.
• Graduate of an accredited vocational nursing program.
• Minimum 1 year of nursing experience (home health preferred).
• Physically able to assist and move patients.
• Strong communication and documentation skills.
• Proficiency with electronic charting and care coordination software.
• Valid driver's license and reliable transportation; willingness to travel to patient homes in Sun City and surrounding Maricopa areas.

Your Role as a Licensed Practical Nurse (LPN)
