Job summary

Trustwell Living, LLC is seeking a Licensed Nurse to provide direct resident care in an in-home health setting and supervise daily activities per individualized care plans. The role reports to the Health Services Director and works to ensure resident safety, privacy, and well-being.

Responsibilities include administering medications, documenting changes, and coordinating with nursing staff to implement care plans while maintaining professional standards and compliance with state regulations.

Qualifications

  • Licensed nurse with at least 1 year in-home health or geriatric nursing experience.
  • CPR/First Aid certification.
  • Maintains appropriate license.

  • Monitors resident health status and signs/symptoms of illness.
  • Administers medications/treatments per community policies/procedures.
  • Implements and monitors resident’s individualized care plan.
  • Documents resident medical data per community policy.
  • Maintains confidentiality of residents, families, and community.
  • Assists residents with daily ADLs.
  • Maintains a safe and clean environment for residents and staff.

Job description

Position Summary

Responsible for providing direct resident care and supervision of residents needs per individualized service/care plan, monitors residents for change of condition and signs/symptoms of illness according to federal and state laws and regulations and according to the policies of the Community. Responsible to communicate and document resident changes or concerns to Health Services Director. Assist in supervision of resident assistants.

Position Responsibilities
  • Complies with all federal and state laws, including state Resident Rights’ law.
  • Complies with all Community policies and procedures
  • Knowledgeable of state regulations pertaining to resident care.
  • Monitors resident health status.
  • Ensures resident privacy and dignity.
  • Assesses, documents and reports change in resident condition.
  • Provides emergency medical intervention as needed.
  • Administers medications/treatments per community policies/procedures.
  • Ensures implementation resident’s individualized care plan.
  • Provides recommendation to DOW for changes to resident care plan.
  • Communicates residents care plan changes to resident assistants.
  • Documents resident medical data per community policy.
  • Documents new physician orders and communicates change to nursing staff.
  • Monitors ADL documentation per community policy.
  • Documents, investigates and timely reports all resident incidents.
  • Immediately intervenes, reports and investigates suspected or alleged verbal, physical, sexual abuse or theft from resident.
  • Maintains safe storage of medication.
  • Reviews monthly weight/vital sign documentation.
  • Implements and monitors behavioral management interventions.
  • Assist residents with daily ADL’s.
  • Maintains confidentiality of residents, families, and community.
  • Maintains a safe and clean environment for residents and staff.
  • Practices universal precautions.
  • Complies and oversees infection control procedures per community policy.
  • Reports safety concerns and maintenance issues to supervisor.

  • Willingness to adhere to assisted living principles in provided services.
  • Ability to utilize and apply basic management principles.
  • Ability to recognize and communicate problems effectively.
  • Ability to relate to the public, Residents, families, staff, other professionals appropriately.
  • Ability to communicate well, verbally and in writing.
  • Ability to maintain accurate records.
  • Ability to adhere to and communicate policy and procedures.
  • Ability/willingness to improve skills of self and staff; to be active professionally
  • Ability to lift to 50 pounds occasionally, 30 pounds regularly.
  • Maintains appropriate and required license.
Training and Experience

Licensed Nurse with a minimum of at least one-year in-home health or geriatric nursing in a similar position. Rehabilitative and restorative experience helpful. CPR/First Aid certification
