Job summary

HarborChase LLC seeks a registered nurse to provide direct, individualized nursing care to assigned residents under the supervision of the Director of Resident Care, complying with state and federal regulations and HarborChase standards.

You will implement physician plans, document care on the MAR, administer medications, assess residents and families, maintain charts, respond to emergencies, and promote safety and resident rights in a team-oriented environment.

Responsibilities

• Provides direct nursing care to assigned residents under the supervision of the Director of Resident Care (DRC).
• Implements plan of care formulated by physician and DRC.
• Provides accurate and timely documentation of care given (MAR).
• Assesses residents and provides information pertinent to diagnosis or plan of care to residents and families.
• Administers medication per physician orders and documents on MAR; monitors for adverse reactions.
• Maintains resident charts and ensures forms are complete and accurate.
• Prepares incident/accident reports and notifies physician/family as required by policy.
• Maintains a safe and healthy working environment and follows clinical standards.
• Assists residents in learning appropriate self-care activities.
• Organizes and monitors resources to meet residents’ needs; ensures safety.
• Recognizes emergencies and initiates appropriate action; protects resident rights.

Position Summary

Provides direct and individual nursing care to assigned residents based on nursing standards and under the supervision of the Director of Resident Care (DRC). Also maintains compliance with all state and federal regulations and the standards of HRA.

Essential Functions

• Ensures quality and safe delivery of health services to residents
• Implements plan of care formulated by physician and DRC
• Provides accurate and timely documentation of care given
• Assesses and provides resident and family/care giver information pertinent to diagnosis or plan of care
• Administers medication per physician orders. Documents properly on MAR. Observes for any adverse reaction and notifies physician accordingly
• Assembles resident charts, resident records and checks that all necessary forms are complete and accurate
• Prepares incident/accident reports for residents as needed per policy. Notifies physician and family post incident/accident per policy and documents appropriately
• Maintains safe and healthy working environment and uses safe work methods and procedures in accordance with clinical standards
• Assists resident in learning appropriate self-care activities
• Organizes and monitors associates and material resources necessary for meeting the needs of the residents under his/her care
• Recognizes emergencies and life-threatening situations and initiates appropriate action(s)
• Maintains confidentiality and resident rights regarding all resident personal information
• Performs any other duties requested by supervisor
• The ability to work in a safe and alert manner
• The ability to take ownership for associate’s safety and the safety of the residents
• Must disclose any medication that might impair associate’s ability to perform the job safely or competently.
