Seeking a compassionate Licensed Practical Nurse (LPN) to deliver resident-centered skilled nursing care. The LPN administers medications and treatments, monitors vital signs, supports rehabilitation goals, and promptly reports any changes in resident condition.

Working closely with RNs, physicians, and therapists, the LPN helps develop and follow individualized care plans and maintains accurate documentation. The role prioritizes residents’ comfort, dignity, and safety, including effective communication with families and collaboration within a multidisciplinary team in a supportive, mission-driven environment.

Key Qualifications & Skills

• Active LPN license


• Strong clinical skills: medication administration, treatments, and vital sign monitoring


• Ability to support rehabilitation and individualized care planning


• Accurate documentation and timely communication of condition changes


• Compassionate, resident-focused approach with strong teamwork and communication skills
