Job summary

Ontario Trillium Foundation is seeking an experienced Licensed Practical Nurse to provide skilled home health care under supervision. You will assess patients, monitor vital signs, document care, and coordinate with physicians and therapists to update care plans.

Responsibilities include educating patients and families, maintaining accurate records, and adapting plans based on patient responses. Strong interpersonal skills and CPR certified are essential for this full-time, on-site role.

Qualifications

  • Associates degree in nursing is required; a Bachelor's degree is preferred.
  • Current and valid Licensed Practical Nurse license.
  • One year of LPN experience.
  • Current CPR certification.
  • Valid driver’s license and auto insurance.
  • Strong interpersonal and communication skills.
  • Computer literacy and good organizational skills.
  • Ability to meet physical demands (lifting up to 50 pounds).

Responsibilities

  • Provide skilled nursing care under supervision.
  • Complete evaluation tasks including vital signs and medication reviews.
  • Coordinate with physicians, therapists and care team.
  • Assess and chart observations of patient condition at each visit.
  • Maintain complete documentation of nursing care.
  • Work collaboratively with health care team.
  • Educate patients and families on home health care procedures.
  • Record patient medical information and vital signs.
  • Monitor and report symptoms or changes in patient condition.
  • Consult with health care team to assess, plan and evaluate care plans.
  • Prepare patients for examinations or treatments.
  • Modify treatment plans as indicated by patient responses.
  • Order, interpret, and evaluate diagnostic tests.
  • Monitor diet and physical activity as part of care.
  • Assess needs of individuals/families/communities and home/work environments.
  • Engage in nursing-related research activities.
  • Perform other duties as assigned.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

  • Provide skilled nursing care under the direct supervision of the clinicalsupervisor
  • Complete evaluation tasks including vital signs and medicationreviews
  • Coordinate with physicians, therapists, and other individuals in the client’s care plan
  • Assess and chart observations of the patient’s condition at eachvisit
  • Maintain complete and accurate documentation of nursingcare
  • Work collaboratively with all members of the health care team
  • Provide education to patients and families on proper home health care procedures andstrategies
  • Record patient’s medical information and vital signs
  • Monitor, record, and report symptoms or changes in patient’s conditions
  • Consult and coordinate with health care team members to assess, plan, implement, or evaluate patient careplans
  • Administer local, inhalation, intravenous, or otheranesthetics
  • Prepare patients for and assist with examinations ortreatments
  • Modify patient treatment plans as indicated by patient’s responses and conditions
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient’s condition
  • Monitor all aspects of patient care, including diet and physicalactivity
  • Assess the needs of individuals, families, or communities, including the assessment of the patient’s home or work environments, to identify potential health or safety problems
  • Engage in research activities related to nursing
  • Performother duties asassigned
Minimum Qualifications (Knowledge, Skills, and Abilities)
  • Associates degree required; Bachelor’s degree from school of nursingpreferred
  • Current and valid Licensed Practical Nurse license
  • One year of LPN experience
  • Current CPR certification required
  • Valid driver’s license required
  • Current automobile registration and insurance required
  • Strong interpersonal skills that build personal rapport and establish trust with the patient
  • Computer literate
  • Strong organizational skills
  • Excellent written and communication skills
  • Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
