Job summary

Consumer Directed Services Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home and community-based nursing care.

You will assess patients, administer medications and treatments, monitor vital signs, document care, and collaborate with RNs and physicians to follow care plans. You will educate individuals and families to promote wellbeing and ensure high-quality outcomes through an interdisciplinary team approach, with emphasis on person-centered care and accurate

Qualifications

• Current LPN licensure required.
• Strong clinical assessment and medication/treatment administration skills.
• Ability to monitor and report vital signs and health status changes.
• Experience supporting care plans with RNs and physicians.

• Proficiency with electronic health record documentation.
