Qualifications

  • Licensed Practical Nurse (LPN) must be licensed and graduate from an approved nursing school.
  • Experience in nursing is preferred, along with CPR and IV certifications.

Responsibilities

Job description

A qualified Licensed Practical Nurse (LPN) administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Registered Nurse (RN). Services are furnished in accordance with Agency policies. In this role, you will report to the Director of Nursing (DON).


Duties/Responsibilities:

  1. Furnishes services, treatments and diagnostic and preventive procedures as assigned per the plan of care, in accordance with agency policies.
  2. Prepares clinical and progress notes.
  3. Assists the physician, the intermediate care provider and RN in performing specialized procedures.
  4. Prepares equipment and materials for treatments observing aseptic technique as required.
  5. Assists the patient in learning appropriate self-care techniques.
  6. Understands and adheres to established policies and procedures.
  7. Follows/implements the nursing care plan for each patient as approved by the RN/physician.
  8. Initiates preventive and rehabilitative nursing procedures as appropriate.
  9. Observes signs and symptoms and reports to the physician and RN nurse reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
  10. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
  11. Evaluates with RN the effectiveness of the LPN's nursing service to the patient and family under the guidance of the RN.
  12. Maintains accurate and complete records of observations, treatments and care of patient.
  13. Participates in medical record audit as assigned.
  14. Attends staff meetings, patient care conferences and in-services as scheduled.
  15. Takes on-call duty, nights, weekends and holidays as assigned.
  16. Submits any changes in schedule to appropriate scheduling position in their office on a daily basis or as applicable by EMR workflow.
  17. Participates in patient care conferences to discuss the need for involvement of other members of the health team, such as physical therapist or speech language pathologist.
  18. Observes aseptic technique and standard precautions as required.
  19. Accepts, writes and executes orders; orders are cosigned by RN where applicable.
  20. Ensures that the patient/family is involved in implementing the plan of care.
  21. Assists in reviewing, evaluating and updating the patient plan of care as appropriate.
  22. Administers medications and treatments as ordered; documents appropriately.
  23. Provides appropriate information regarding patient to other agencies/ individuals involved in patient's care as needed and as directed by the RN.
  24. Observes confidentiality and safeguards all patient related information (PHI) in accordance with HIPAA requirements.
  25. Completes documentation and paperwork in a timely manner as per Agency policy.
  26. Immediately reports/documents to Nursing Supervisor/RN any patient incidents/variances or complaints.
  27. Demonstrates competent performance of technical skills according to established procedures before performing new skills.
  28. Participates in peer review, utilization review and QAPI activities as assigned.
  29. Provide effective communication to patient/family, team members, and other health care professionals.
  30. Understands and adheres to established state, federal and/or accreditation policies/procedures.
  31. Adheres to Company standards and consistently interprets and accurately performs all assigned responsibilities.
  32. Maintains acceptable attendance status, per Company policy.
  33. Reports all incomplete work assignments to Nursing Supervisor/RN.
  34. Appearance is always within Agency standards; is clean and well groomed.
  35. Demonstrates effective time management skills through daily documentation and infrequent overtime for routine assignments.
  36. Maintains clean and neat work environment.
  37. Demonstrates sound judgment, critical thinking and decision making.
  38. Performs other duties as assigned.

Required Skills/Abilities/Knowledge:
  • Average computer skills with competency in media communications.
  • Must have the physical abilities to perform the described duties.
  • Acceptance of vision, philosophy and goals of the Company.
  • Ability to exercise initiative and independent judgment.
  • Excellent verbal and written communication skills.

Education/Experience/Licenses/Certifications:
  • Graduate of an approved school of practical nursing and currently licensed in the state(s) in which practicing.
  • One year experience in nursing (preferred).
  • Active and valid CPR certification with practicum.
  • IV certification by state mandate, if required by the state.

Pay Range: $27.00 - $35.00 / hour
