Job summary

Granite Peak Home Health in Great Falls, MT is seeking an LPN to join our home health team. You will implement care plans, provide nursing services, and support patients in their homes under supervision of an RN and Home Health Director.

The role requires a licensed Practical Nurse with at least one year of nursing experience and a commitment to our agency’s goals and patient-centered care.

Qualifications

• Graduate of a state-approved practical nursing program and licensed in the practicing state.
• Minimum of one year nursing experience preferred.
• Acceptance of philosophy and goals of this Agency.
• Ability to exercise initiative and independent judgment.

Responsibilities

• Understands and adheres to established policies and procedures.
• Implements the nursing care plan for each patient.
• Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
• Initiates preventive and rehabilitative nursing procedures as appropriate.
• Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient’s condition.
• Teaches and counsels the patient and family regarding nursing care needs.
• Evaluates nursing service effectiveness under RN guidance.
• Maintains accurate and complete patient records.
• Participates in medical record audits.
• Attends staff meetings and care conferences.
• Takes on-call duty, nights, weekends, and holidays as assigned.
• Prepares clinical and progress notes.
• Assists physician and RN in performing specialized procedures.
• Prepares equipment and materials for treatments.
• Observe aseptic technique as required.
• Assists the patient in learning self-care techniques.

Principal Responsibilities:

This position reports to the Home Health Director

1. Understands and adheres to established policies and procedures.
2. Implements the nursing care plan for each patient.
3. Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
4. Initiates preventive and rehabilitative nursing procedures as appropriate.
5. Observe signs and symptoms and report to the physician and RN reactions to treatments, including drugs and changes in the patient’s physical or emotional condition.
6. Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
7. Evaluates with registered nurses the effectiveness of the LPN’s/LVN’s nursing service to the patient and family under the guidance of the registered nurse.
8. Maintains accurate and complete records of observations, treatments, and care of patients.
9. Participates in medical record audit as assigned.
10. Attends staff meetings, patient care conferences, and in-servicess as scheduled.
11. Takes on-call duty, nights, weekends, and holidays as assigned.
12. Is responsible for:
1. Submitting any changes in schedule to the Director of Clinical Services/Clinical Manager daily.
2. Participating in patient care conferences to discuss the need for the involvement of other members of the health team, such as physical therapists or speech-language pathologists
13. Prepares clinical and progress notes.
14. Assists the physician and RN in performing specialized procedures.
15. Prepares equipment and materials for treatments.
16. Observe the aseptic technique as required.
17. Assists the patient in learning appropriate self-care techniques.
